The Kentucky Immunization Registry has launched a public facing portal that allows individuals to log in and access their own or their child’s immunization records. Individuals will be able to access their/their child’s full immunization record, COVID-19 Vaccine Specific record with a SMART Health Card QR code, and school certificate.

HEALTH REQUIREMENTS
3 and 4 YEAR OLD PROGRAM
1. Kentucky Certificate of Immunization:
All children are required to present a valid up-to-date immunization certificate upon enrollment in school.
KRS 214. 034 requires any child enrolled as a regular attendee in all public or private primary or secondary schools, and preschool programs shall have a current immunization certificate and be on file within two weeks of the child’s attendance. Information including school immunization legislation and regulations, school immunization schedules, school immunization reporting forms and requirements can be found at the
Kentucky Immunization Program Web site.
Contact your child's physician, primary care provider, or the Green River District Health Department for an updated Immunization Certificate.
2. Preventative Health Care Examination:
702 KAR 1:160 requires each child first entering a Kentucky public school or entering Grade 6 to present a Kentucky Preventative Health Care Examination Form. This exam may be completed up to one year prior to initial entry. Click

3. Eye Exam for School Entry:
KRS 156.160 (1)(i) requires proof of a vision examination by an optometrist or ophthalmologist. This evidence shall be submitted to the school no later than January 1 of the first year that a child ages 3, 4, 5 or 6 is enrolled in public school, public preschool, or Head Start program.
INITIAL ENTRY IN SCHOOL (Kindergarten – Grade 12)
1. A Kentucky Certificate of Immunization:

Age-specific immunization requirement and one booster dose requirement:
- 2-Dose Series of HepA (Age: 12 months through18 years)
- Quadrivalent meningococcal vaccine (MenACWY) booster dose (Age: 16 years)

4. Dental Exam/Screening
Kentucky law
KRS 156.160 (j) requires proof of a dental screening or examination by a dentist, dental hygienist, physician, registered nurse, nurse practitioner, or physician assistant to be presented to the school no later than January 1 of the first year that a child age 5 or 6 is enrolled in public school. SIXTH GRADE REQUIREMENTS:
1. Preventative Examination:
A second examination is required within one year prior to entry into Grade 6.
This shall be reported on the “Preventative Health Care Examination Form – 6th Grade."
2. Kentucky Immunization Certificate:
A valid up-to-date immunization certificate with the mandated Tdap booster, one dose of Meningococcal, second dose of Varicella vaccine needs to be on file at the school for enrollment. Requirements can be found at the

TRANSFER STUDENTS
1. Preventative Health Care Examination:

2. Kentucky Immunization Certificate:
A valid up-to-date Kentucky immunization certificate is required. Out-of-state immunization certificates may be accepted if they meet the same age-specific requirements as outlined in this regulation. The DCPS district nurse may be able to transfer out-of-state shot records to a Kentucky form. Requirements can be found at the
Kentucky Immunization Program web site.
VALID IMMUNIZATION EXEMPTION CERTIFICATES
The following are considered VALID immunization exemption certificates:
Certificate of Religious Exemption
KRS 214.036 requires parents who object to immunization of their child to provide a written sworn statement objecting to immunization of the child on religious grounds. Click HERE for form.
Certificate of Medical Exemption
These certficates must be completed and signed by a physician or Health Department official.
